Passport clinic dates are announced for 2015

Six passport clinics will be offered again this year by MP Dave Van Kesteren. The first is scheduled for Wednesday April 8th at Blessed Sacrament Church, 145 Victoria Avenue in Chatham. It
will be held between the hours of 10 a.m. and 2 p.m.

The remaining clinics are as follows:
Wednesday, May 20 - Leamington
(Kinsmen Recreation Complex - Gym)
Wednesday, June 24 - Ridgetown
(St. Michael’s Parish Hall)
Wednesday, July 22 - Tilbury
(Knights of Columbus Hall)
Wednesday, August 19 - Blenheim
(Glad Tidings Community Church - Gym)
Wednesday, September 2 - Chatham
(Blessed Sacrament Parish Hall)

The cost of an adult 5 year passport is $120.00 while the 10 year option is $160.0. The children’s passports
up to age 15 are $57.00. Payment can be made either by major credit card or certified cheque or money
order payable to the Receiver General for Canada.

Participants can pick up the new applications upon arrival at the clinic or in advance at any post office.
Volunteers will be on hand to assist those needing help in completing their application and to ensure the
application is fully completed before mailing. A photographer will be at the clinic to take passport photos
and copying is available for any documents to be submitted. Passport photos cost $18.00 (including tax),
payable in cash.
Please note that passports are NOT issued at these clinics. The Member of Parliament’s office provides
mailing during the week following the clinic. This allows participants several days to complete
requirements if needed.
